Wrist pain is a common ailment that many individuals may experience due to strain, injuries, or certain health conditions such as rheumatoid arthritis, an inflammatory autoimmune condition that commonly affects smaller joints, including the wrists and hands. Although some cases of wrist pain may subside on their own, persistent wrist pain can cause great discomfort and hinder you from carrying out daily activities. Traditional Chinese Medicine (TCM) offers a holistic approach for wrist pain management, focusing on underlying imbalances and restoring normal function from within. This comprehensive guide explores wrist pain and the various treatments available.

Causes of Wrist Pain from TCM Perspective

In TCM, each part of the body is not viewed in isolation, but treated as part of a network with the body’s overall energy system. When the wrist experiences ailment or pain, it is often seen as a sign of imbalance in the body’s vital energy, (Qì 气), and blood (Xuè 血). The common causes of wrist pain according to TCM may include:

1. Qi and Blood Stagnation

Repetitive movements of the wrist may cause tension or injuries that hinder a smooth flow of Qi and blood. This prevents adequate nourishment for the wrist, which may lead to pain and swelling.

2. External Wind and Dampness

When the body is imbalanced due to a poor diet or an unhealthy lifestyle, it becomes more susceptible to external factors, such as cold wind or damp weather (风寒湿). This may cause stiffness and discomfort in the wrist meridians.

3. Internal Deficiencies

Various deficiencies, such as weakened liver, kidneys, or spleen, may contribute to a decrease in overall wellbeing. This may eventually lead to stagnation, weakness, and vulnerability in different parts of the body, including the wrists.

4. Injuries or Weakness

Bruising, swelling, or other trauma on the wrist may be another cause of wrist pain. Moreover, overusing your wrists for typing or heavy lifting can further weaken the tendons and ligaments, causing discomfort and reduced flexibility.

Types of Wrist Pain

In TCM, wrist pain falls under the category of Bi Zheng (痹症), which includes chronic pain, stiffness, and reduced mobility caused by Qi and blood stagnation. Here are some types of wrist pain from a TCM perspective.

1. Early Stage: Wind and Cold Dampness Type (风寒湿型)

The initial stage of wrist pain is considered to be caused by wind and cold dampness (风寒湿型), characterised by joint pain particularly during cold, humid or rainy weather.

2. Prolonged Stage: Yin Deficiency Type (久病化热伤阴型)

This type of wrist pain may be accompanied by a burning sensation or swelling, and is typically linked to a Yin Deficiency condition. Yin Deficiency is known to cause excessive internal heat in the body, which causes discomfort and pain.

3. Chronic Stage: Phlegm and Blood Stasis Obstruction Type (痰瘀痹阻型)

This is a condition where the pain is more persistent and intense, significantly limiting mobility when carrying out daily activities. This occurs due to long-term stagnation of Qi and blood, leading to the buildup of phlegm that blocks the meridians. It is common among individuals with arthritis or advanced Carpal Tunnel Syndrome, who spend long hours doing repeated hand or wrist movements, causing pain, tingling, and numbness.

Wrist Pain Relief Treatment

Based on the underlying cause and type of wrist pain that you have, your TCM practitioner may recommend various treatments to relieve your symptoms.

Acupuncture

Acupuncture is a popular TCM treatment for wrist pain. It involves inserting tiny, sterile needles at key acupuncture points or acupressure points, which may help clear internal blockages of the blood and Qi circulation, providing pain relief for the wrist. This is often prescribed for conditions like carpal tunnel syndrome or tendon overuse.

For wrist pain, key acupressure points may include:

  • Yangchi (TB-4 阳池): May help to relax tendons and boost Qi
  • Yangxi (LI-5 阳溪): May help to dispel wind and inflammation
  • Tongli (HT-5 通里): May help activate the wrist channel and alleviate pain
  • Shousanli (LI-10 手三里): May help to treat weaknesses in the arm and wrist
  • Wenliu (LI-7 温溜): May help to clear the heat and relieve acute pain

Chinese Herbal Medicine

Aside from acupuncture, Chinese herbal medicines may also be prescribed in an integrated treatment plan to help support recovery and pain relief. The herbs may be consumed in pill, powder, or tea form, and are chosen for their anti-inflammatory and pain-relieving properties, including:

  • Danggui 当归 (Angelica sinensis): May help invigorate blood and soothe pain
  • Guizhi 桂枝 (Cinnamomum cassia): May enhance circulation and reduce inflammation
  • Duhuo 独活 (Angelica pubescens Maxim): May help dispel wind-damp and alleviate joint pain
  • Baiwei 白薇 (Radix Cynanchi Atrati): May help to clear heat and detoxify the blood

Gua Sha

Gua Sha (刮痧) is a TCM therapy where a smooth-edged tool is used to scrape the skin, helping to promote blood circulation and relieve any muscle tension. For wrist pain, Gua Sha is applied along the meridians connecting to your wrist, such as Lung Meridian (LU-9 / 太渊, Taiyuan), Pericardium Meridian (PC-6 / 内关 Neiguan), and Triple Energiser (TA-5 / 外关, Waiguan), which also helps reduce inflammation and discomfort.

Cupping Therapy

For wrist pain caused by stiffness or overuse, cupping therapy or 拔罐疗法 may be recommended to relax tight forearm and wrist muscles. This treatment utilises glass, bamboo, or plastic cups to create suction on the skin, which may also stimulate the flow of Qi.

Moxibustion

Moxibustion is a TCM wrist pain treatment which involves burning dried, ground mugwort leaves (Ai Ye or 艾叶) near or on specific points on your body. The gentle warmth helps promote Qi flow and reduce stiffness, especially for cold-type or chronic wrist pain.
